Medical oversight is provided through Dr. Lynn Bourdon and Physician Assistant, Kristi Nealy, who provide the daily care to our patients. Services include: evaluation and treatment of acute and chronic illness; diagnostic radiology and laboratory services; minor emergency triage; and immunizations. Case Managers, counselors, and other support staff provide access to a full array of support services including care coordination, community advocacy, community education, prescription assistance, transportation and linkages to other community resources.
