Financial Assistance

Financial assistance is available to individuals and families whose income is between 101% and 200% of the Federal Poverty Income Guidelines and who have no insurance. Those whose income falls at or below the 100% guideline are assessed a minimal fee for the office visit and other procedures done at the time of the visit. Applications for the Sliding Fee Scale Discount are available at the front desk.

Proof of income and proof of family composition is required prior to accessing discounted fees.

Fee Policy:

• Payment is expected at the time of service.**
• The minimum fee is expected for each visit.
• All labs and X-rays must be paid on the day of service.
