Your First Visit

How to prepare:

We want your first visit to be as stress free as possible. We suggest that you prepare for that first visit by:

  1. Arrive 15 to 20 minutes early to the appointment to complete initial forms.
  2. Proof of insurance card.
  3. Social Security Card, if available.
  4. Your driver’s license or other proof of identity.
  5. Bring your current medications.
  6. If you are applying for a discounted fee or sliding fee, proof of income, proof of Texas Residency, and proof of family relationships must be provided.
    • Proof of income can be your last year’s income tax face page or last months paystubs or a letter from your employer that states your income (if paid in cash).
    • Proof of Texas Residency, such as a copy of the previous month’s utility bill.
    • Documentation of proof of family relationships, such as, student report card(s), copy of birth certificate, baptismal record for each child in the family.
